Children’s International Medical Group has partnered with The American Academy of Pediatrics (AAP) and the National Center for Medical Implementation (NCHMI) to provide your child with the gold standard of healthcare that they are entitled to. This is accomplished through the implementation of the Medical Home Program in all Children’s International Medical Group locations.

What is Medical Home?

Medical Home is not a singular, physical location, rather it is a healthcare model devoted to providing comprehensive, high-quality primary care that exists in all locations, and is represented through a network of clinical specialists, families, and community resources that are all centered around your child.

What Can You Expect at Medical Home Facilities?

The primary goal of Medical Home facilities, including Children’s International Medical Group, is to provide the highest quality care in the shortest amount of time. With that being said, Medical Home facilities are focused on helping you manage your child’s healthcare, answering your health questions, listening to your concerns, networking with other medical professionals if needed, coordinating your child’s care with additional services, and encouraging you and your child to be actively involved in their health care.

What is the Role of the Medical Care Team?

Your child’s Medical Home care team will treat you and your child as partners in your child’s care. They will take ample time to learn more about your child, your family, your life situation, and you and your child’s preferences.

During office visits, your child’s team will reserve time to have a conversation about your child’s options for care. This conversation will begin with a thorough explanation of the options, followed by an opportunity for questions, and additional clarification if needed.

Finally, your child’s Medical Home team will support you in caring for your child. It is the priority of your team to help you set and reach goals for your child’s care and provide you with any additional resources to help your child stay healthy.

What is Your Role as Part of Your Child’s Medical Home Team?

As part of your child’s Medical Home team, Children’s International Medical Group invites you to be an active participant in your child’s healthcare approach. To remain an essential component in advocating for your child’s health, you should bring a list of questions to each appointment, as well as a list of any medications, vitamins, or remedies you use for your child. Finally, you can assist your child’s Medical Home team by providing a clear means of communication when you don’t understand something, when your child receives services from additional healthcare providers, or when you are not satisfied with your child’s care.
